Attia's strength model is organized around three principles rather than a lift list. First, always train both phases: the concentric force a muscle produces as it shortens, and the eccentric force it produces as it lengthens. The eccentric phase is what decelerates you, and it is where aging people fail — the falls happen coming down stairs or off a curb, not going up. Second, hinge at the hip without necessarily loading the spine. Squats and deadlifts both transmit force down the spinal axis and need coaching to do safely; a step-up delivers the hinge with no axial loading, works one leg at a time so asymmetries surface, and isolates the eccentric beautifully on the step-down. Third, carry. Attia wants patients able to hold half their body weight in each hand and walk for a minute, and notes grip strength appears repeatedly as a longevity proxy.

Adopt both phases as principle number one
Attia's first strength-training principle is always doing both concentric and eccentric work — not necessarily in the same movement or equally on a given day, but never letting the eccentric go untrained.
Watch out The eccentric phase is equally important and oft ignored, which is precisely why it becomes the failure point.

Slow the negatives
The simplest implementation is accentuating the lowering — something like two seconds up and four seconds down on a trap bar deadlift. Focus as much on the negative as on the positive.

Add dedicated eccentric movements
Nordic curls, where you kneel with feet held and lower yourself slowly, are purely eccentric. Attia calls it eccentric misery and says it requires real starting strength.
Pro tip Start assisted; most people lack the hamstring strength to do it unaided.
Watch out Do not program this out of the gate for an untrained person.

Make step-ups the staple hinge
A step-up is a single-leg hip hinge with no axial loading, and the step-down isolates the eccentric. Attia considers it possibly the single most important movement for most people.
Pro tip If you cannot control the descent, you are using too much resistance.

Train the carry to a standard
Work toward carrying half your body weight in each hand and walking for a full minute. Grip strength shows a monotonic relationship with dementia onset and dementia death in Attia's cited figure.
Pro tip Attia argues from the Bradford Hill criteria that training grip strength likely improves outcomes, not merely marks them.

Test with the broad jump
Attia checks that his standing broad jump exceeds his own height — at five foot ten, at least six feet. It is an extreme combination of concentric explosion and eccentric deceleration in one movement.
Pro tip He credits Andy Galpin for the metric and checks it constantly.
Watch out Landing a maximal broad jump is a genuine eccentric ask; do not test it cold or untrained.
In the wild
Attia points to stairs as the everyday case. Walking up is concentrically taxing, but the injuries happen walking down or stepping off a curb. Watch aging people and the visible difficulty is in slowing themselves, not in propelling themselves. Falls and broken hips follow eccentric weakness far more than concentric weakness. That single observation is what promotes eccentric training from an accessory concern to the first principle of his strength programming.

Attia says he is so far down the step-up rabbit hole he can no longer easily explain the nuances of oblique compression and pelvic angles, or the differences between how his left and right leg perform the movement. Because it is single-leg, the exercise surfaces asymmetries everyone has but few detect under a barbell. Tinkering with it, he says, lets him work out many of the kinks in how his body moves.

Common mistakes
Training only the obvious phase
Concentric force is what people feel themselves producing, so the deceleration half of every movement quietly goes untrained.
Assuming hinging means squats and deadlifts
Both load the spinal axis and require coaching to perform safely. Hip thrusters, lunges and step-ups deliver the hinge without that load.
Believing sport substitutes for resistance
Attia is blunt that daily tennis and walking are good but do nothing for type 2 fibres, which are what you lose first with age.

Adults training for physical independence into old age rather than for a gym number.
Not ideal for
Novices with no coaching access attempting heavy axially loaded lifts, or anyone with an unresolved injury requiring rehab first.
