The Leucine-Anchored Protein Protocol

Attia's protocol has three dials. The first is quantity: about one gram per pound of current body weight, which for him at roughly 180 pounds means 180 grams a day. He is explicit that at moderate body fat the distinction between current, lean, and target body weight does not matter enough to fuss over. The second is distribution: two 90-gram boluses do not accomplish what four 45-gram servings do. Each serving needs to clear roughly 20 to 25 grams so the amino acids are not simply routed into gluconeogenesis, and his reading of the literature puts the upper edge around 50 grams. The third is quality: instead of fixating on total grams, track leucine, lysine and methionine, the three amino acids that matter most for hypertrophy and the ones plant protein is thinnest in.
Origin
In autumn 2021 a DEXA scan showed Attia had substantially less muscle than a decade earlier, which he attributed largely to years of extreme fasting. He rebuilt his protein approach around quantity, timing and amino acid quality and gained 13 to 14 pounds of lean mass in about twelve months.

How to run it

Measure before you change anything
Attia uses periodic DEXA scans and plots the trend, watching appendicular lean mass index and fat-free mass index rather than body fat alone. ALMI divides arm and leg lean tissue in kilograms by height in metres squared.
Pro tip Attia wants his ALMI at the 90th percentile or above; the data favour anything north of the 75th.

Set the daily target
One gram per pound of current body weight. At about 15 percent body fat, Attia says the lean-versus-total distinction is not worth agonizing over.

Split into four servings inside the window
Spread the daily total across roughly four servings rather than one or two large ones. Each serving should sit above about 20 to 25 grams and below about 50.
Pro tip Below the floor, the liver diverts the amino acids to gluconeogenesis instead of structure.
Watch out Attia calls the upper limit 'a bit of an unclear limit' — 50 grams is his best reading, not a hard number.

Track the three amino acids, not just grams
Focus on leucine, lysine and methionine per meal or per day. Attia targets roughly 6 to 8 grams of leucine, a comparable amount of lysine, and at least 2 grams of methionine.
Pro tip Hitting the amino acid targets almost assuredly drags total protein to the right place.

Choose sources for bioavailability
Animal protein is more bioavailable. Cooking plant protein frees some of it from fibre, but a 20 to 30 percent haircut remains. Eggs and dairy make vegetarian targets far easier, eggs being high in methionine.
Pro tip Ferriss notes Bill Pearl became Mr Universe as a lacto-ovo vegetarian; excluding all animal products is a different problem from excluding meat.

Drop intra-workout BCAAs for post-workout whole protein
Attia stopped using branched-chain amino acids in workouts. They do not persist long in free form, the dose is too small, and the synthesis window is broad enough that a large protein meal within a couple of hours works.
Pro tip He now drinks only an electrolyte drink during training and takes high-quality whey afterwards.
In the wild
A DEXA scan in the autumn of 2021 shocked Attia: at low-to-mid 170s body weight he had far less muscle than ten years earlier. He traced it to three years of aggressive fasting — up to ten days water-only roughly quarterly, seven-day fasts routinely, three-day fasts monthly — while still lifting daily. Without amino acids coming in, the training was tearing down muscle it could not rebuild. He changed protein quantity, timing and quality and adjusted training toward hypertrophy, and gained 13 to 14 pounds of lean mass in about twelve months without anabolic steroids.
→ 13 to 14 pounds of lean mass added in roughly twelve months, which Attia describes as not Herculean but a matter of priority.
Common mistakes
Two big boluses instead of four servings
Attia is explicit that two 90-gram servings do not accomplish nearly what four 45-gram servings do. Same total, different result.
Relying on BCAAs during training
They do not stick around long enough in free form, the quantity is too low, and timing within the workout does not appear to matter given how broad the synthesis window is.
Fasting hard while training hard
Attia lifted daily through extended water-only fasts and still lost lean mass, because stimulus without substrate is net catabolic.
Is it for you?
Best for
Anyone deliberately trying to add or defend lean mass, especially as they age past 40.
Not ideal for
People with kidney disease or other conditions where protein load is medically restricted, and anyone unwilling to track intake.
